Missouri communities
St. Louis City is its own jurisdiction, separate from the county. The directory includes Downtown, the Central West End, North St. Louis and South St. Louis.
St. Louis County wraps around the city. Its pages cover North County (Florissant, Ferguson, Hazelwood), central county (Clayton, University City, Ladue), West County (Chesterfield, Ballwin, Creve Coeur, Wildwood) and South County (Affton, Mehlville, Oakville).
Across the Missouri River, St. Charles County includes St. Charles, St. Peters, O’Fallon, Wentzville and Lake St. Louis. To the south, Jefferson County includes Arnold, Imperial, Festus and Crystal City. Farther west, Franklin County includes Washington.
Illinois communities in the Metro East
Across the Mississippi, the Metro East takes in St. Clair County (Belleville, O’Fallon, Fairview Heights) and Madison County (Edwardsville, Glen Carbon, Collinsville, Granite City, Alton, Godfrey), along with Monroe and Clinton counties.
The state line matters for practical reasons. Clinicians are licensed state by state, and Missouri and Illinois each run their own Medicaid program. Tell us which state the person lives in, so the request goes to a treating practice that works there.
Care settings across the region
Where the person lives is half the question. The setting is the other half. Care can be arranged in:
- Private homes, through mobile wound care and in-home primary care
- Skilled nursing facilities and rehabilitation centers
- Assisted living and memory care communities
- Outpatient or office settings with clinical partners
Gateway’s administrative office is in Chesterfield. It is not a clinic, and patients are not seen there.
How to check your address
Call 314-325-0126, or use the request care form, which asks only for a name and phone number. When we talk, tell us the city or ZIP code and where the person is living now. Availability is confirmed by location and care setting, so we do not promise timing in advance.

Common questions
My town is not in the directory. Can I still ask?
Yes. The directory does not list every town where care can be arranged. If you live in or near Greater St. Louis or the Metro East, call 314-325-0126 with the address. We check with the treating practices that work in that part of the region and tell you whether a visit can be arranged there.
Does Gateway have a clinic I can go to?
No. The office in Chesterfield is administrative only. Care happens where the person lives or is already receiving care: a private home, a skilled nursing facility, assisted living, memory care or a rehabilitation center. Some partner clinicians also see patients in outpatient or office settings. When that is the better fit, we give you the location and can help with transportation arrangements.
Is care arranged the same way in Illinois as in Missouri?
The steps are the same. You call or send a request, we gather what the treating practice needs, and the visit is arranged for your location. What can differ is the treating practice and the coverage questions, because clinicians are licensed by state and plans vary between Missouri and Illinois. Keep the insurance card handy so those questions can be checked for the right state.
Can a facility in an outlying county set up regular visits?
Facilities anywhere in the service area can ask. Skilled nursing, assisted living, memory care and rehabilitation teams can call about one resident or about a recurring schedule. As with home visits, availability is confirmed for the building’s location. Tell us the facility name, the town and roughly how many residents need wound follow-up.
