Why Gateway exists

Recovery continues long after a patient leaves the hospital. A wound may need repeated assessment, treatment and coordination among several clinicians. Transportation, limited mobility and changing care settings can make that follow-up harder to arrange.

Gateway helps bring appropriate care closer to the patient, while keeping families, nursing teams and existing physicians involved.

Built around the people doing the caring

We work with hospital social workers and discharge planners, skilled nursing facilities and independent physician practices. Patients, families and caregivers can also contact us directly for help finding the next step.

Physician leadership, local relationships

Medical Director Mohamad Zanbrakji, MD, oversees clinical standards and the wound-care program. Our leadership team brings together clinical direction, regional operations and community partnerships.
