Contact Gateway with the anticipated discharge date, destination and care needs. We review whether follow-up can be arranged and explain which records or orders are needed.
A clear handoff
The discharge plan, current wound treatment and responsible clinicians guide the next step. Gateway coordinates with the sending team, receiving facility or home-health agency so each team understands its role.
After the move
Follow-up considers changes in the wound, treatment needs and support available in the new setting. Findings are shared with the patient’s existing clinicians as appropriate.
Confirm the arrangements
A referral request does not confirm acceptance or a visit. Our team reviews location, clinical needs, coverage and availability before agreeing on next steps.
