Information submitted through forms
Gateway's public request and referral forms collect contact information, role or relationship, general location, care setting, high-level wound concern, and a brief message. The forms are intended to help Gateway contact the sender and determine the appropriate next step.
Do not submit detailed medical records
Do not use public website forms or general email to submit wound photographs, test results, full medical records, Social Security numbers, financial account information, or other highly sensitive information. Gateway may provide an appropriate secure channel or fax workflow when documents are needed.
How information is used
Information is used to respond to requests, review potential service fit, coordinate referrals, answer questions, and improve website operations. Gateway may share information with personnel or service providers who support these functions, subject to applicable privacy and security requirements.
Website analytics and technical data
The hosting provider may process standard technical information such as IP address, browser type, device type, pages visited, and timestamps for security, reliability, and analytics. Gateway does not sell website contact information.
