NP-led home wound care across Washington, Union, Pacific, Sullivan, St. Clair, and New Haven. Mobile specialty care for rural and semi-rural Missouri communities.
Franklin County sits to the west of the St. Louis metro and is one of the regions where access to specialty wound care is genuinely limited. Residents of Washington, Union, Pacific, Sullivan, St. Clair, and New Haven often face long drives to reach an outpatient wound clinic — and for chronic wounds requiring weekly dressing changes, that simply isn’t sustainable.
Gateway Wound Care closes that gap. Our nurse practitioners travel to Franklin County homes, assisted living facilities, and skilled nursing communities with the same protocols and standards as a metro wound center. We treat diabetic foot ulcers, venous leg ulcers, pressure injuries, post-surgical wounds, and complex wounds requiring NPWT.
We partner with Mercy Washington, area primary care offices, home-health agencies, and discharge planners to support patients after hospital stays and to prevent avoidable readmissions.
Franklin County cities and communities where Gateway Wound Care delivers visits:
Franklin County’s assisted living and skilled nursing communities are critical partners for Gateway Wound Care. We bring NP-level wound management on-site to facility residents — reducing the burden of off-site clinic appointments and supporting in-place aging.
Our nurse practitioners work as an extension of your nursing staff — performing wound assessments, bedside debridement, NPWT management, and post-surgical wound care. All visits are documented with photos and shared with the facility record and referring physician.
