Our nurse practitioners serve patients discharged from Barnes-Jewish, SSM Saint Louis University Hospital, Mercy South, and other St. Louis hospitals — as well as residents of the city’s assisted living communities and skilled nursing facilities. We treat diabetic foot ulcers, venous leg ulcers, pressure injuries, post-surgical wounds, and any complex wound needing consistent specialty care.
We work closely with primary care offices, home-health agencies, and discharge planners across the city to reduce readmissions and keep patients at home. Every visit is documented and reported back to the referring physician.
