  • Agree on a main person and a backup for each task, with the patient's preferences guiding what is shared.

  • Keep the latest clinician instructions and contact information in one agreed place that authorized helpers can access.
  • For each handoff, record what was completed, what is still pending, and which questions have already been sent to the team.

Be clear about limits

  • Tell the clinician when a task is beyond a helper's training, comfort, time, or physical ability, and ask for guidance.
  • If family members remember instructions differently, bring the question to the treating team rather than creating a new plan together.
