- Skin changes may include persistent discoloration, unusual warmth, firmness, tenderness or an open area.
- Ask the care team to explain the individual plan for mobility, pressure relief, skin checks and support surfaces.
- Seek prompt medical advice for worsening pain, pus, fever or hot, swollen skin around the wound.
