Bring the resident's priorities
- Start with what the resident wants the team to understand, including comfort, privacy, communication, and daily routines.
- Write down a few concrete questions and ask how the resident would like family or other supporters involved.
Understand each person's role
- Ask who oversees the wound plan, who carries out day-to-day care, and which facility contact can help with questions.
- If an outside wound clinician is involved, ask how recommendations and updates are shared with the facility team.
Confirm the follow-through
- Summarize the agreed next steps, the person responsible for each one, and how the resident or authorized family member will receive updates.
- Ask whom to approach if an agreed action is unclear or a concern has not been addressed.
