Discharge is a transition, not the end of care.

Gateway helps turn a written discharge plan into a visible next step. We coordinate appropriate wound and medical follow-up, clarify contacts and keep the primary care physician and referring team connected.

Clinical boundaries

Some concerns need urgent or specialty care.

Gateway will say clearly when emergency care, vascular evaluation, surgery, podiatry or another service is the safer next step. Call 911 for urgent or life-threatening symptoms.
