Primary and total care

A current medication list supports safer care.

After a hospital, rehabilitation or specialist visit, Gateway can review the available medication lists, clarify questions with the care team and keep the established physician involved.

Why it matters

Medication continuity belongs inside a coordinated plan.

Gateway considers the immediate concern, the patient’s health, the established care team and the setting where daily care takes place.

See the full contextCurrent symptoms, wound status, recent transitions, chronic conditions and practical barriers are reviewed together.
Work togetherGateway works alongside the existing physician, agency, facility and caregiver.
Keep the plan clearThe next visit, communication plan and specialty needs remain easy to understand.

Clinical boundaries

Some concerns need urgent or specialty care.

Gateway will say clearly when emergency care, vascular evaluation, surgery, podiatry or another service is the safer next step. Call 911 for urgent or life-threatening symptoms.
