The Referral Process
Step 1: Submit Referral
Providers can submit referrals by phone at (314) 804-8364, fax, email, or through our online referral form. Include basic patient information, wound type and location, relevant medical history, and insurance information.
Step 2: Insurance Verification
We verify insurance coverage and authorization requirements. We contact the patient or facility to confirm location and schedule the initial assessment.
Step 3: Initial Assessment
Our specialist evaluates the wound, reviews medical history, and develops a comprehensive treatment plan. We photograph the wound for documentation and progress tracking.
Step 4: Ongoing Communication
We provide detailed written reports to you after the initial assessment and regularly thereafter. We notify you immediately of any significant changes in patient status, complications, or deviation from expected healing trajectory.
Step 5: Discharge and Follow-up
When the wound has healed or the patient's care goals have been achieved, we provide a summary report and recommendations for ongoing preventive care to reduce recurrence.
